"The ISSUE 2 study evaluated the effectiveness of risk stratification and diagnosis of NMS solely based on the ESC guidelines on syncope. Therapy is delayed until documentation of syncopal event by the ILR. Half of the patients had asystole at the time of syncope, indicating that pacing might be effective. Criticisms included the high number of ILRs to be implanted to diagnose one syncopal event. However, TT and ATP testing fail to predict the mechanism of spontaneous NMS, except for asystole during TT testing. Non-syncopal arrhythmic ECG findings correlate with the mechanism of spontaneous syncope."
